NEW CLIENT DOCUMENTS INSTRUCTIONS
All clients must complete and submit intake paperwork 24 hours before their first session. If your forms
 are not received by then, the appointment will be cancelled and rescheduled after paperwork is received. 

Call 505-243-2551 if you have any problems accessing / completing forms electronically or have any other questions


OPTIONS TO SUBMIT PAPERWORK: Email, Fax, or Regular Mail

When you click on the buttons below, a pdf file will be displayed (you may be asked to open with Adobe Acrobat). DOWNLOAD document to your computer or Mobile Phone / Tablet. You can then do one of 
the following:

1. Type information  directly into document, save it in electronic format with your name as the title, and EMAIL it back to us at info@outcomesnm.org 

2. You can also print forms, fill out manually and FAX them to 505-243-0446 - OR - MAIL forms to:
 1503 University Blvd NE Albuq., NM 87102 

If you are not able to do either  of the above, we can MAIL PAPERWORK for you to complete and return 
to us either by FAX to 505-243-0446 - OR - MAIL to 1503 University Blvd NE Albuq., NM 87102 

* Clients who have Aetna, ComPsych, Empathia, Espyr, or Magellen should choose Insurance / Self-pay

* Release of Information is only necessary if you need records released to you or another party

la siguiente:


1. Escriba la información directamente en el documento, guárdela en formato electrónico con su nombre como título y envíenosla por correo electrónico a info@outcomesnm.org


2. También puede imprimir formularios, completarlos manualmente y enviarlos por FAX al 505-243-0446 - O - ENVIAR POR CORREO POSTAL a:

Si no puede hacer ninguna de las cosas anteriores, podemos ENVIARLE LA DOCUMENTACIÓN POR CORREO para que la complete y la devuelva.

* Los clientes que tienen Aetna, ComPsych, Empathia, Espyr o Magellen deben elegir Seguro/Pago por cuenta propia
